INSTRUCTIONS:
This form is to be completed by a parent/guardian (or by the student if they are 18 years or older).

Due to the ongoing pandemic, only minimal essential course changes will be accommodated this year.  Only exceptional circumstances that meet one or more of the following criteria will be considered:

-Course level change (e.g., Academic to Applied)
-Missing post-secondary prerequisite or graduation requirement (Grades 11 and 12 only)
-Improperly sequenced prerequisite(s)
-Course scheduled that has already been completed (e.g., Summer School)

We will not be able to accommodate requests for teacher or cohort preferences, or to simply reorder the sequence of your courses.  We anticipate that most requests will not be possible given the current restrictions around physical distancing and health and safety guidelines.  We thank you in advance for your understanding.

The deadline submit course change requests is the week prior to the start of the new Quadmester.  For example, the deadline to make a change to a Quadmester 3 course is the end of the week prior to the start of Quadmester 3.  After that time, students may no longer add a new course for their upcoming Quadmester, but eligible Grade 11 or 12 students may request to drop a course in favour of a spare at any point during the school year.

On this form you will use the drop-down menus to select the courses YOU WANT (NOT the courses your child currently has).  The purpose of this form is for you to see if the requested course change is possible, given the order of the timetable.  This form is NOT to be used to rearrange existing courses, and preference requests (e.g., to keep existing courses but to switch from one English class to another) will not be accommodated.

TIMELINES:
Requests are processed in the order in which they are received.  A member of the Student Services Department will e-mail you after your request has been processed.  During the school week, we endeavour to process changes within 1-2 school days.

WAITING LISTS:
To ensure public health and safety guidelines and reduce points of contact, students may not add/start a new course after a new Quadmester has started.  If a course is full, we will add your child's name to a course waiting list .  If space in a course prior to the start of that course Quadmester, we will make the change and contact you.  While we cannot predict movement of course waiting lists, the majority of students get their requested courses.  If space does not become available, your child's timetable will remain as is.

Please note that changes made by your Guidance Counsellor are confirmed, even though the changes will not be updated in Xello for one week.

ONTARIOCOLLEGS.CA/OUAC WEBSITES: *For Grade 12 students only*
The OntarioColleges.ca/OUAC post-secondary application services will automatically update the courses listed in the Education/Academic Information section of your profiles the first week of the new quadmester.

By submitting this request you indicate your understanding of the following:

6. The final decision regarding timetable changes rests with the Principal.
